Due to the complexity of a natural-language processing, the user is often constrained to a set of available commands. In messenger-like apps, the user selects them through a graphical user interface. Chatbots can also be regarded as an evolutionary step in the design and development of human-computer interaction. Whereas the common desktop metaphor replaced the command-line decades ago, the ongoing convergence of smart, mobile technologies and the power of artificial intelligence and machine learning enables a user-friendly control of computers through the natural language. The language can be thought of as a natural user interface (NUI). The word “natural” implies the possibility to take advantage of a user’s innate skills, reducing the difference between a novice and long-time user. Therefore, chatbots as NUI are “leveraging the potential of modern technologies to better mirror human capabilities” (Wigdor &amp; Wixon, p. 10), significantly lowering the entry barriers, costs and time devoted to learning a new product or service. When compared to virtual reality or Xbox Kinect, the Wigdor and Wixon’s call for using the modern input technology does not play a central part in chatbot development; however, their potential to accelerate the full integration of technology into our lives and fulfil the promise of ubiquitous computing is certainly present. With chatbots we are on the brink of a next paradigm shift, from the design of graphical layouts to AI-powered software where a conversation is simultaneously the medium and content of user experiences. We can call it a “conversational interface” (Knight 2016).\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Ca id=\"the-rise-of-chatbots\" name=\"the-rise-of-chatbots\">\u003C\u002Fa>\u003C\u002Fp>\n\u003Ch2>\u003Cspan style=\"font-weight: 400;\">The rise of chatbots\u003C\u002Fspan>\u003C\u002Fh2>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">Chatbots represent a point where two prominent features of human culture intersect: the emphasis on the verbal expression which is unique to humans, and the post-industrial logic of design and production, structured around the just-in-time delivery, personalized content, one-to-one communication, and adjusting based on the immediate context. We find all these features in any typical conversation. The conversational element of chatbots is also stressed by the industry. Satya Nadella and Microsoft announced their commitment to chatbots, stating that the rise of chatbots “is inevitable” (Pall) and describe “a conversation as a platform”. (Microsoft News Center). The attributes critical to a successful conversation are, among other things, the sensitivity to contextual clues and delivering content that is meaningful and useful. \u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">Digital technology and designers have been answering these demands since the beginning of personal computing. The human-computer interaction field, or HCI, arose from the need to study and propose solutions to ease the use of computer technology. During its first-wave, the HCI field brought about developments in understanding of cognitive aspects of computer use that eventually materialised in usability engineering guidelines and suggestions. After the advent of mobile computing — in a mainstream culture largely recognized as the introduction of Apple iPhone in 2007 — the HCI field as well as interaction designers had to take into account the temporal and spatial dimensions of technology use, thus accepting the broader changes in human-computer relationship, where technology moved from a periphery of specialized problem-solving domains to the central positions as a cultural artefact. Apps, which developers crafted with mobile context and experiences in mind, started to dominate. Web designers reacted accordingly, discovering the mix of front-end technologies and browser-supported functionalities that allowed the Web platform to become “responsive” to the variety of browsers, display sizes, and different contextual capabilities of devices. The proof-of-concept itself was introduced in 2010. Following three years, the concept became a trend, when a well-known site in the industry, Mashable, called 2013 the year of responsive web design. (Murtagh) Finally, in the 1Q of 2015 the fraction of mobile functionality, the messaging, overshadowed not only the the other mobile apps, but also the social networking apps. Therefore chatbots are new digital music or apps, where the equivalences to iTunes and Apple store platforms are Facebook Messenger, Amazon’s Alexa, Skype, Telegram, Kik, Slack and other upcoming solutions to the chatbot-enabled platforms. To asses the “burgeoning bot economy”, Business Insider summarizes its report on chatbots by stating that:\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">“Chatbots could be lucrative for messaging apps and the developers who build bots for these platforms, similar to how app stores have developed into moneymaking ecosystems.” (Beaver)\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">The mentioned tech companies provide a shop-like list of bots, except for Facebook and its Messenger, which has not released its own centralised shop to date, although the profiles of prominent users such as CNN or Barack Obama already provide a chatbot-enhanced messaging, whose capabilities range from news to music suggestions.\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Ca id=\"chatbots-in-digital-health\" name=\"chatbots-in-digital-health\">\u003C\u002Fa>\u003C\u002Fp>\n\u003Ch2>\u003Cspan style=\"font-weight: 400;\">Chatbots in Digital Health\u003C\u002Fspan>\u003C\u002Fh2>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">The digital health turn that reshapes the ingrained processes of medical practice has delivered so far many smart devices bundled with biosensors, mobiles apps analysing data and other digital services. While these solutions offer personalised data and suggestions, the real disruptive step seems to come from the interaction of the new digital ecosystems and the old, albeit well-established, and necessarily conservative environment of the public healthcare systems. It is for this transition that chatbots may play a lead role by embodying the function of a virtual assistant, bridging the gap between patients and clinicians. \u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">Powered by AI and machine learning algorithms, chatbots are forecasted to save “health care costs when used in place of a human, such as a preliminary step of helping to assess a condition and providing self-care recommendations” („The chatbot will see you now: AI may play doctor in the future of healthcare“ 2016) Such prediction is reinforced by the recent successful attempts to automate diagnosis of mental health issues by the analysis of speech patterns with 100% predictive power. (Bedi, et al. 2015)\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">The digital health field is already saturated with chatbots. The company Your.MD provides a chatbot solution based on the machine-learning algorithms and natural-language processing that predicts probabilities between the symptoms and conditions, which are analysed on the background of a user’s personal profile. („The chatbot will see you now: AI may play doctor in the future of healthcare“ 2016)\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">TigerConnect offers secure, encrypted, HIPAA compliant messaging platform and chatbots, helping to foster communication between clinicians and patients by providing EHR\u002FEMR records management, physician appointments, patient adherence etc.\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">Another high-profile chatbot deployment came from the announcement of National Health Service in the UK, which intends to betatest chatbots with \u003C\u002Fspan>\u003Cspan style=\"font-weight: 400;\">Babylon, a UK-based telemedicine startup, supporting video conferencing with doctors, in order to “ replace NHS 111, a non-emergency phone hotline staffed by call center workers who aren’t necessarily medical professionals”. („UK’s NHS will test Babylon’s triage chatbot to replace non-emergency hotline“ 2017)\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">Among many others stands out the Amazon’s Alexa-based app created with the cooperation of Boston Children’s Hospital called KidsMD. Compared to the previously mentioned chatbot solutions relying on the text-based messages, KidsMD is a voice-recognition app that has been tested at patients’ homes and hospitals as well. Parents can used the app to obtain information about common illnesses and quick treatments, all of which is curated by doctors and accessible on the cloud. The app also helps doctors to take notes and photographs with crucial metadata while operating. (Bailey 2016)\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Ca id=\"-summary\" name=\"-summary\">\u003C\u002Fa>\u003C\u002Fp>\n\u003Ch2>\u003Cspan style=\"font-weight: 400;\">Summary\u003C\u002Fspan>\u003C\u002Fh2>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">The quest of Digital Health for optimizing the Patient Engagement Experience (PEx) and the general process of interacting with patients will benefit most from adopting chatbots and voice-recognition apps in the cases where the preliminary assessment is routine enough to be automated. The possibilities to streamline a patient-doctor communication exist as we could see in the examples such as KidsMD or Babylon’s trial with NHS in the UK. The more chatbots will become employed in the whole digital and mobile universe, the more familiar they will be to patients willing to try them as alternatives. However, the direct link to doctors will not go away and is currently being deployed in several projects. If the Babylon-NHS cooperation succeeds, other major transformations in public sector health may follow, stimulating the whole Digital Health industry. \u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">Apart from the technical aspects, chatbots are a service and as such directly intertwined with human beings, their skills, fears, prejudices and cognitive spans, which means that chatbots as any service must be designed with a human in mind. It has been said that from the perspective of User interface design, the development of chatbots is less concerned with the visual, and focuses more on the narrative, conversational dimensions. Chatbots pose new challenges to the whole HCI\u002FUX community, which requires their reorientation to the new platform. Therefore, we can expect a time delay between the available technological opportunities and appropriate design solutions that will emerge some time later, after designers test and find functional design patterns that work for all the stakeholders.\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">Finally, we have to assume that the extreme use cases and users-outliers will constitute major dilemmas as far as ethical and legal issues are concerned. What should chatbot do in case of a dying patient? What would be the legal ramifications when the bug occurs and causes health damages? que logiciels, les chatbots reconnaissent les messages textuels ou vocaux d’un utilisateur et y répondent de manière appropriée. En raison de la complexité du traitement du langage naturel, l’utilisateur est souvent limité à un ensemble de commandes disponibles. Dans les applications de type messagerie, il les sélectionne au moyen d’une interface graphique. Les chatbots peuvent aussi être considérés comme une étape évolutive dans la conception et le développement de l’interaction humain-machine. Alors que la métaphore habituelle du bureau a remplacé la ligne de commande il y a des décennies, la convergence actuelle des technologies mobiles intelligentes et de la puissance de l’intelligence artificielle et de l’apprentissage automatique permet de commander les ordinateurs de façon conviviale par le langage naturel. Le langage peut être pensé comme une interface utilisateur naturelle (NUI). Le mot « naturel » implique la possibilité de tirer parti des aptitudes innées de l’utilisateur, en réduisant l’écart entre le novice et l’utilisateur de longue date. Les chatbots en tant que NUI « exploitent » donc « le potentiel des technologies modernes pour mieux refléter les capacités humaines » (Wigdor &amp; Wixon, p. 10), en abaissant sensiblement les barrières à l’entrée, les coûts et le temps consacré à l’apprentissage d’un nouveau produit ou service. Comparé à la réalité virtuelle ou à la Kinect de la Xbox, l’appel de Wigdor et Wixon à utiliser les technologies d’entrée modernes ne joue pas un rôle central dans le développement des chatbots ; leur potentiel d’accélérer la pleine intégration de la technologie dans nos vies et de tenir la promesse de l’informatique ubiquitaire est cependant bien présent. Avec les chatbots, nous sommes au seuil d’un nouveau changement de paradigme, de la conception de mises en page graphiques vers des logiciels propulsés par l’IA, où la conversation est à la fois le médium et le contenu des expériences utilisateur. Nous pouvons l’appeler une « interface conversationnelle » (Knight 2016).\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Ca id=\"the-rise-of-chatbots\" name=\"the-rise-of-chatbots\">\u003C\u002Fa>\u003C\u002Fp>\n\u003Ch2>\u003Cspan style=\"font-weight: 400;\">L’essor des chatbots\u003C\u002Fspan>\u003C\u002Fh2>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">Les chatbots représentent le point où se croisent deux traits marquants de la culture humaine : l’accent mis sur l’expression verbale, propre à l’être humain, et la logique postindustrielle de la conception et de la production, structurée autour de la livraison juste à temps, du contenu personnalisé, de la communication individuelle et de l’adaptation au contexte immédiat. Nous retrouvons tous ces traits dans n’importe quelle conversation ordinaire. La dimension conversationnelle des chatbots est également soulignée par l’industrie. Satya Nadella et Microsoft ont annoncé leur engagement en faveur des chatbots, déclarant que leur essor « est inévitable » (Pall), et décrivent « la conversation comme plateforme » (Microsoft News Center). Les qualités décisives pour une conversation réussie sont, entre autres, la sensibilité aux indices contextuels et la fourniture d’un contenu qui ait du sens et soit utile. \u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">La technologie numérique et les designers répondent à ces exigences depuis les débuts de l’informatique personnelle. Le domaine de l’interaction humain-machine, ou HCI, est né du besoin d’étudier et de proposer des solutions pour faciliter l’usage de la technologie informatique. Au cours de sa première vague, le champ de la HCI a fait progresser la compréhension des aspects cognitifs de l’usage de l’ordinateur, qui s’est finalement concrétisée dans les lignes directrices et les recommandations de l’ingénierie de l’utilisabilité. Après l’avènement de l’informatique mobile — que la culture grand public associe largement au lancement de l’iPhone d’Apple en 2007 —, le champ de la HCI comme les designers d’interaction ont dû prendre en compte les dimensions temporelles et spatiales de l’usage de la technologie, acceptant ainsi les changements plus larges de la relation humain-ordinateur, où la technologie est passée de la périphérie des domaines spécialisés de résolution de problèmes à une position centrale d’artefact culturel. Les applications, que les développeurs façonnaient en pensant au contexte et aux expériences mobiles, ont commencé à dominer. Les designers web ont réagi en conséquence, en découvrant le mélange de technologies front-end et de fonctionnalités prises en charge par les navigateurs qui a permis à la plateforme Web de devenir « responsive », c’est-à-dire de s’adapter à la variété des navigateurs, des tailles d’écran et des différentes capacités contextuelles des appareils. La preuve de concept elle-même a été présentée en 2010. Au cours des trois années suivantes, le concept est devenu une tendance, lorsqu’un site bien connu du secteur, Mashable, a proclamé 2013 l’année du responsive web design. Les chatbots sont donc la nouvelle musique numérique ou les nouvelles applications, et les équivalents des plateformes iTunes et App Store sont Facebook Messenger, Alexa d’Amazon, Skype, Telegram, Kik, Slack et d’autres solutions à venir parmi les plateformes ouvertes aux chatbots. Pour évaluer l’« économie florissante des bots », Business Insider résume son rapport sur les chatbots en affirmant que :\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">« Les chatbots pourraient être lucratifs pour les applications de messagerie et pour les développeurs qui créent des bots pour ces plateformes, de la même manière que les magasins d’applications sont devenus des écosystèmes rentables. » (Beaver)\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">Les entreprises technologiques citées proposent une liste de bots à la manière d’une boutique, à l’exception de Facebook et de son Messenger, qui n’a pas encore lancé sa propre boutique centralisée, bien que les profils d’utilisateurs de premier plan comme CNN ou Barack Obama offrent déjà une messagerie enrichie de chatbots, dont les capacités vont des actualités aux suggestions musicales.\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Ca id=\"chatbots-in-digital-health\" name=\"chatbots-in-digital-health\">\u003C\u002Fa>\u003C\u002Fp>\n\u003Ch2>\u003Cspan style=\"font-weight: 400;\">Les chatbots dans la santé numérique\u003C\u002Fspan>\u003C\u002Fh2>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">Le tournant de la santé numérique, qui remodèle les processus enracinés de la pratique médicale, a produit jusqu’ici de nombreux appareils intelligents équipés de biocapteurs, des applications mobiles qui analysent des données et d’autres services numériques. Si ces solutions offrent des données et des recommandations personnalisées, le véritable pas disruptif semble venir de l’interaction entre les nouveaux écosystèmes numériques et l’environnement ancien, bien que solidement établi, et nécessairement conservateur des systèmes publics de santé. C’est dans cette transition que les chatbots pourraient jouer un rôle de premier plan, en incarnant la fonction d’assistant virtuel qui comble le fossé entre patients et cliniciens. \u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">Propulsés par l’IA et par des algorithmes d’apprentissage automatique, les chatbots devraient, selon les prévisions, faire économiser « des coûts de santé lorsqu’ils sont utilisés à la place d’un être humain, par exemple comme étape préliminaire pour aider à évaluer un état et à fournir des recommandations de soins personnels » (« The chatbot will see you now: AI may play doctor in the future of healthcare » 2016). Cette prévision est renforcée par les tentatives récentes et réussies d’automatiser le diagnostic de troubles de santé mentale par l’analyse des schémas de parole, avec un pouvoir prédictif de 100 %. (Bedi, et al. 2015)\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">Le champ de la santé numérique est déjà saturé de chatbots. L’entreprise Your.MD propose une solution de chatbot fondée sur des algorithmes d’apprentissage automatique et sur le traitement du langage naturel, qui prédit les probabilités entre symptômes et pathologies, analysées sur le fond du profil personnel de l’utilisateur. (« The chatbot will see you now: AI may play doctor in the future of healthcare » 2016)\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">TigerConnect propose une plateforme de messagerie sécurisée, chiffrée et conforme à la HIPAA ainsi que des chatbots, qui aident à favoriser la communication entre cliniciens et patients en assurant la gestion des dossiers EHR\u002FEMR, des rendez-vous médicaux, de l’observance des patients, etc.\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">Un autre déploiement de chatbots très médiatisé est venu de l’annonce du National Health Service (NHS) au Royaume-Uni, qui entend tester en version bêta des chatbots avec \u003C\u002Fspan>\u003Cspan style=\"font-weight: 400;\">Babylon, une start-up britannique de télémédecine qui permet des visioconférences avec des médecins, afin de « remplacer le NHS 111, une ligne téléphonique pour les cas non urgents, tenue par des employés de centre d’appels qui ne sont pas nécessairement des professionnels de santé ». (« UK’s NHS will test Babylon’s triage chatbot to replace non-emergency hotline » 2017)\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">Parmi beaucoup d’autres se distingue l’application KidsMD, fondée sur Alexa d’Amazon et créée en coopération avec le Boston Children’s Hospital. Contrairement aux solutions de chatbots mentionnées plus haut, qui reposent sur des messages textuels, KidsMD est une application de reconnaissance vocale, testée au domicile des patients ainsi qu’à l’hôpital. Les parents peuvent utiliser l’application pour obtenir des informations sur les maladies courantes et les traitements rapides, le tout étant sélectionné par des médecins et accessible dans le cloud. L’application aide aussi les médecins à prendre des notes et des photographies accompagnées de métadonnées essentielles pendant qu’ils opèrent. (Bailey 2016)\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Ca id=\"-summary\" name=\"-summary\">\u003C\u002Fa>\u003C\u002Fp>\n\u003Ch2>\u003Cspan style=\"font-weight: 400;\">Conclusion\u003C\u002Fspan>\u003C\u002Fh2>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">La quête de la santé numérique pour optimiser l’expérience d’engagement du patient (Patient Engagement Experience, PEx) et le processus général d’interaction avec les patients tirera le plus grand profit de l’adoption de chatbots et d’applications de reconnaissance vocale dans les cas où l’évaluation préliminaire est assez routinière pour être automatisée. Les possibilités de fluidifier la communication entre patient et médecin existent, comme nous avons pu le voir dans des exemples tels que KidsMD ou l’essai de Babylon avec le NHS au Royaume-Uni. Plus les chatbots seront employés dans tout l’univers numérique et mobile, plus ils deviendront familiers aux patients disposés à les essayer comme alternative. Le lien direct avec les médecins ne disparaîtra cependant pas, et il est actuellement mis en place dans plusieurs projets. Si la coopération entre Babylon et le NHS réussit, d’autres transformations majeures de la santé publique pourraient suivre, stimulant toute l’industrie de la santé numérique. \u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">Au-delà des aspects techniques, les chatbots sont un service et, à ce titre, directement entrelacés avec les êtres humains, leurs compétences, leurs peurs, leurs préjugés et leurs capacités cognitives, ce qui signifie que les chatbots, comme tout service, doivent être conçus en pensant à l’être humain. On a dit que, du point de vue du design d’interface utilisateur, le développement de chatbots se soucie moins du visuel et se concentre davantage sur les dimensions narratives et conversationnelles. Les chatbots posent de nouveaux défis à toute la communauté HCI\u002FUX, qui doit se réorienter vers la nouvelle plateforme.
