Due to the complexity of a natural-language processing, the user is often constrained to a set of available commands. In messenger-like apps, the user selects them through a graphical user interface. Chatbots can also be regarded as an evolutionary step in the design and development of human-computer interaction. Whereas the common desktop metaphor replaced the command-line decades ago, the ongoing convergence of smart, mobile technologies and the power of artificial intelligence and machine learning enables a user-friendly control of computers through the natural language. The language can be thought of as a natural user interface (NUI). The word “natural” implies the possibility to take advantage of a user’s innate skills, reducing the difference between a novice and long-time user. Therefore, chatbots as NUI are “leveraging the potential of modern technologies to better mirror human capabilities” (Wigdor &amp; Wixon, p. 10), significantly lowering the entry barriers, costs and time devoted to learning a new product or service. When compared to virtual reality or Xbox Kinect, the Wigdor and Wixon’s call for using the modern input technology does not play a central part in chatbot development; however, their potential to accelerate the full integration of technology into our lives and fulfil the promise of ubiquitous computing is certainly present. With chatbots we are on the brink of a next paradigm shift, from the design of graphical layouts to AI-powered software where a conversation is simultaneously the medium and content of user experiences. We can call it a “conversational interface” (Knight 2016).\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Ca id=\"the-rise-of-chatbots\" name=\"the-rise-of-chatbots\">\u003C\u002Fa>\u003C\u002Fp>\n\u003Ch2>\u003Cspan style=\"font-weight: 400;\">The rise of chatbots\u003C\u002Fspan>\u003C\u002Fh2>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">Chatbots represent a point where two prominent features of human culture intersect: the emphasis on the verbal expression which is unique to humans, and the post-industrial logic of design and production, structured around the just-in-time delivery, personalized content, one-to-one communication, and adjusting based on the immediate context. We find all these features in any typical conversation. The conversational element of chatbots is also stressed by the industry. Satya Nadella and Microsoft announced their commitment to chatbots, stating that the rise of chatbots “is inevitable” (Pall) and describe “a conversation as a platform”. (Microsoft News Center). The attributes critical to a successful conversation are, among other things, the sensitivity to contextual clues and delivering content that is meaningful and useful. \u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">Digital technology and designers have been answering these demands since the beginning of personal computing. The human-computer interaction field, or HCI, arose from the need to study and propose solutions to ease the use of computer technology. During its first-wave, the HCI field brought about developments in understanding of cognitive aspects of computer use that eventually materialised in usability engineering guidelines and suggestions. After the advent of mobile computing — in a mainstream culture largely recognized as the introduction of Apple iPhone in 2007 — the HCI field as well as interaction designers had to take into account the temporal and spatial dimensions of technology use, thus accepting the broader changes in human-computer relationship, where technology moved from a periphery of specialized problem-solving domains to the central positions as a cultural artefact. Apps, which developers crafted with mobile context and experiences in mind, started to dominate. Web designers reacted accordingly, discovering the mix of front-end technologies and browser-supported functionalities that allowed the Web platform to become “responsive” to the variety of browsers, display sizes, and different contextual capabilities of devices. The proof-of-concept itself was introduced in 2010. Following three years, the concept became a trend, when a well-known site in the industry, Mashable, called 2013 the year of responsive web design. Therefore chatbots are new digital music or apps, where the equivalences to iTunes and Apple store platforms are Facebook Messenger, Amazon’s Alexa, Skype, Telegram, Kik, Slack and other upcoming solutions to the chatbot-enabled platforms. To asses the “burgeoning bot economy”, Business Insider summarizes its report on chatbots by stating that:\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">“Chatbots could be lucrative for messaging apps and the developers who build bots for these platforms, similar to how app stores have developed into moneymaking ecosystems.” (Beaver)\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">The mentioned tech companies provide a shop-like list of bots, except for Facebook and its Messenger, which has not released its own centralised shop to date, although the profiles of prominent users such as CNN or Barack Obama already provide a chatbot-enhanced messaging, whose capabilities range from news to music suggestions.\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Ca id=\"chatbots-in-digital-health\" name=\"chatbots-in-digital-health\">\u003C\u002Fa>\u003C\u002Fp>\n\u003Ch2>\u003Cspan style=\"font-weight: 400;\">Chatbots in Digital Health\u003C\u002Fspan>\u003C\u002Fh2>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">The digital health turn that reshapes the ingrained processes of medical practice has delivered so far many smart devices bundled with biosensors, mobiles apps analysing data and other digital services. While these solutions offer personalised data and suggestions, the real disruptive step seems to come from the interaction of the new digital ecosystems and the old, albeit well-established, and necessarily conservative environment of the public healthcare systems. It is for this transition that chatbots may play a lead role by embodying the function of a virtual assistant, bridging the gap between patients and clinicians. \u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">Powered by AI and machine learning algorithms, chatbots are forecasted to save “health care costs when used in place of a human, such as a preliminary step of helping to assess a condition and providing self-care recommendations” („The chatbot will see you now: AI may play doctor in the future of healthcare“ 2016) Such prediction is reinforced by the recent successful attempts to automate diagnosis of mental health issues by the analysis of speech patterns with 100% predictive power. (Bedi, et al. 2015)\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">The digital health field is already saturated with chatbots. The company Your.MD provides a chatbot solution based on the machine-learning algorithms and natural-language processing that predicts probabilities between the symptoms and conditions, which are analysed on the background of a user’s personal profile. („The chatbot will see you now: AI may play doctor in the future of healthcare“ 2016)\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">TigerConnect offers secure, encrypted, HIPAA compliant messaging platform and chatbots, helping to foster communication between clinicians and patients by providing EHR\u002FEMR records management, physician appointments, patient adherence etc.\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">Another high-profile chatbot deployment came from the announcement of National Health Service in the UK, which intends to betatest chatbots with \u003C\u002Fspan>\u003Cspan style=\"font-weight: 400;\">Babylon, a UK-based telemedicine startup, supporting video conferencing with doctors, in order to “ replace NHS 111, a non-emergency phone hotline staffed by call center workers who aren’t necessarily medical professionals”. („UK’s NHS will test Babylon’s triage chatbot to replace non-emergency hotline“ 2017)\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">Among many others stands out the Amazon’s Alexa-based app created with the cooperation of Boston Children’s Hospital called KidsMD. Compared to the previously mentioned chatbot solutions relying on the text-based messages, KidsMD is a voice-recognition app that has been tested at patients’ homes and hospitals as well. Parents can used the app to obtain information about common illnesses and quick treatments, all of which is curated by doctors and accessible on the cloud. The app also helps doctors to take notes and photographs with crucial metadata while operating. (Bailey 2016)\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Ca id=\"-summary\" name=\"-summary\">\u003C\u002Fa>\u003C\u002Fp>\n\u003Ch2>\u003Cspan style=\"font-weight: 400;\">Summary\u003C\u002Fspan>\u003C\u002Fh2>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">The quest of Digital Health for optimizing the Patient Engagement Experience (PEx) and the general process of interacting with patients will benefit most from adopting chatbots and voice-recognition apps in the cases where the preliminary assessment is routine enough to be automated. The possibilities to streamline a patient-doctor communication exist as we could see in the examples such as KidsMD or Babylon’s trial with NHS in the UK. The more chatbots will become employed in the whole digital and mobile universe, the more familiar they will be to patients willing to try them as alternatives. However, the direct link to doctors will not go away and is currently being deployed in several projects. If the Babylon-NHS cooperation succeeds, other major transformations in public sector health may follow, stimulating the whole Digital Health industry. \u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">Apart from the technical aspects, chatbots are a service and as such directly intertwined with human beings, their skills, fears, prejudices and cognitive spans, which means that chatbots as any service must be designed with a human in mind. It has been said that from the perspective of User interface design, the development of chatbots is less concerned with the visual, and focuses more on the narrative, conversational dimensions. Chatbots pose new challenges to the whole HCI\u002FUX community, which requires their reorientation to the new platform. Therefore, we can expect a time delay between the available technological opportunities and appropriate design solutions that will emerge some time later, after designers test and find functional design patterns that work for all the stakeholders.\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">Finally, we have to assume that the extreme use cases and users-outliers will constitute major dilemmas as far as ethical and legal issues are concerned. What should chatbot do in case of a dying patient? What would be the legal ramifications when the bug occurs and causes health damages? software, los chatbots reconocen los mensajes de texto o de voz de un usuario y responden a ellos de forma adecuada. Debido a la complejidad del procesamiento del lenguaje natural, el usuario suele estar limitado a un conjunto de comandos disponibles. En las aplicaciones de tipo messenger, los selecciona a través de una interfaz gráfica de usuario. Los chatbots también pueden considerarse un paso evolutivo en el diseño y el desarrollo de la interacción persona-ordenador. Mientras que la habitual metáfora del escritorio sustituyó a la línea de comandos hace décadas, la actual convergencia de las tecnologías móviles inteligentes y la potencia de la inteligencia artificial y el aprendizaje automático permite un control de los ordenadores fácil de usar mediante el lenguaje natural. El lenguaje puede entenderse como una interfaz natural de usuario (NUI, natural user interface). La palabra «natural» implica la posibilidad de aprovechar las habilidades innatas del usuario, reduciendo la diferencia entre un novato y un usuario veterano. Por tanto, los chatbots en tanto que NUI «aprovechan el potencial de las tecnologías modernas para reflejar mejor las capacidades humanas» (Wigdor &amp; Wixon, p. 10), y reducen de forma significativa las barreras de entrada, los costes y el tiempo dedicado a aprender un nuevo producto o servicio. En comparación con la realidad virtual o Xbox Kinect, el llamamiento de Wigdor y Wixon a utilizar las tecnologías de entrada modernas no desempeña un papel central en el desarrollo de chatbots; sin embargo, su potencial para acelerar la plena integración de la tecnología en nuestras vidas y cumplir la promesa de la ubiquitous computing está sin duda presente. Con los chatbots estamos al borde de un nuevo cambio de paradigma, del diseño de layouts gráficos a un software impulsado por la IA en el que la conversación es a la vez el medio y el contenido de las experiencias de usuario. Podemos llamarlo «interfaz conversacional» (Knight 2016).\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Ca id=\"the-rise-of-chatbots\" name=\"the-rise-of-chatbots\">\u003C\u002Fa>\u003C\u002Fp>\n\u003Ch2>\u003Cspan style=\"font-weight: 400;\">El auge de los chatbots\u003C\u002Fspan>\u003C\u002Fh2>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">Los chatbots representan un punto en el que se cruzan dos rasgos destacados de la cultura humana: el énfasis en la expresión verbal, exclusiva del ser humano, y la lógica postindustrial del diseño y la producción, estructurada en torno a la entrega just-in-time, el contenido personalizado, la comunicación one-to-one y la adaptación al contexto inmediato. Todos estos rasgos los encontramos en cualquier conversación típica. El elemento conversacional de los chatbots lo subraya también la propia industria. Satya Nadella y Microsoft anunciaron su apuesta por los chatbots, afirmando que el auge de los chatbots «es inevitable» (Pall), y describen «la conversación como plataforma». (Microsoft News Center). Entre los atributos decisivos para una conversación satisfactoria están, entre otras cosas, la sensibilidad a las pistas contextuales y la entrega de contenido significativo y útil. \u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">La tecnología digital y los diseñadores llevan respondiendo a estas exigencias desde los inicios de la informática personal. El campo de la interacción persona-ordenador, o HCI (human-computer interaction), surgió de la necesidad de estudiar y proponer soluciones que facilitaran el uso de la tecnología informática. Durante su primera ola, el campo de la HCI trajo avances en la comprensión de los aspectos cognitivos del uso del ordenador que acabaron materializándose en pautas y recomendaciones de ingeniería de la usabilidad (usability engineering). Tras la llegada de la informática móvil —que en la cultura mainstream se identifica en gran medida con la presentación del iPhone de Apple en 2007—, tanto el campo de la HCI como los diseñadores de interacción tuvieron que tener en cuenta las dimensiones temporal y espacial del uso de la tecnología, aceptando así los cambios más amplios en la relación entre el ser humano y el ordenador, en la que la tecnología pasó de la periferia de los ámbitos especializados de resolución de problemas a posiciones centrales como artefacto cultural. Empezaron a dominar las aplicaciones que los desarrolladores creaban pensando en el contexto y las experiencias móviles. Los diseñadores web reaccionaron en consecuencia y descubrieron la combinación de tecnologías front-end y funcionalidades admitidas por los navegadores que permitió a la plataforma web volverse «responsive» ante la variedad de navegadores, tamaños de pantalla y distintas capacidades contextuales de los dispositivos. La prueba de concepto (proof-of-concept) en sí se presentó en 2010. En los tres años siguientes, el concepto se convirtió en tendencia, cuando un conocido sitio del sector, Mashable, declaró 2013 el año del diseño web responsive. UU. pasaban más tiempo usando aplicaciones en sus dispositivos móviles que usando cualquier otra cosa en ordenadores de sobremesa. (Murtagh) Por último, en el primer trimestre de 2015 una parte de la funcionalidad móvil, la mensajería, eclipsó no solo a las demás aplicaciones móviles, sino también a las aplicaciones de redes sociales. Los chatbots son, pues, la nueva música digital o las nuevas aplicaciones, y los equivalentes de las plataformas iTunes y App Store son Facebook Messenger, Alexa de Amazon, Skype, Telegram, Kik, Slack y otras soluciones emergentes de plataformas compatibles con chatbots. Para valorar la «floreciente economía de los bots», Business Insider resume su informe sobre los chatbots afirmando que:\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">«Los chatbots podrían ser lucrativos para las aplicaciones de mensajería y para los desarrolladores que crean bots para estas plataformas, de manera similar a como las tiendas de aplicaciones se han convertido en ecosistemas rentables». (Beaver)\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">Las empresas tecnológicas mencionadas ofrecen un listado de bots a modo de tienda, con la excepción de Facebook y su Messenger, que hasta la fecha no ha lanzado su propia tienda centralizada, aunque los perfiles de usuarios destacados, como la CNN o Barack Obama, ya ofrecen una mensajería enriquecida con chatbots, cuyas capacidades van desde las noticias hasta las recomendaciones musicales.\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Ca id=\"chatbots-in-digital-health\" name=\"chatbots-in-digital-health\">\u003C\u002Fa>\u003C\u002Fp>\n\u003Ch2>\u003Cspan style=\"font-weight: 400;\">Los chatbots en la salud digital\u003C\u002Fspan>\u003C\u002Fh2>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">El giro hacia la salud digital, que está transformando los procesos arraigados de la práctica médica, ha traído hasta ahora muchos dispositivos inteligentes equipados con biosensores, aplicaciones móviles que analizan datos y otros servicios digitales. Mientras que estas soluciones ofrecen datos y recomendaciones personalizados, el paso verdaderamente disruptivo parece provenir de la interacción entre los nuevos ecosistemas digitales y el entorno antiguo, aunque bien asentado y necesariamente conservador, de los sistemas sanitarios públicos. Es precisamente en esta transición donde los chatbots pueden desempeñar un papel protagonista al encarnar la función de un asistente virtual que salva la distancia entre pacientes y clínicos. \u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">Se prevé que los chatbots impulsados por la IA y por algoritmos de aprendizaje automático ahorren «costes sanitarios cuando se usen en lugar de una persona, por ejemplo como paso preliminar que ayuda a evaluar una dolencia y a ofrecer recomendaciones de autocuidado» («The chatbot will see you now: AI may play doctor in the future of healthcare» 2016). Esta predicción se ve reforzada por los recientes intentos exitosos de automatizar el diagnóstico de problemas de salud mental mediante el análisis de patrones del habla con un poder predictivo del 100 %. (Bedi, et al. 2015)\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">El campo de la salud digital ya está saturado de chatbots. La empresa Your.MD ofrece una solución de chatbot basada en algoritmos de aprendizaje automático y procesamiento del lenguaje natural que predice las probabilidades entre síntomas y dolencias, analizadas sobre el trasfondo del perfil personal del usuario. («The chatbot will see you now: AI may play doctor in the future of healthcare» 2016)\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">TigerConnect ofrece una plataforma de mensajería segura, cifrada y conforme con la HIPAA, así como chatbots, que ayudan a fomentar la comunicación entre clínicos y pacientes al ofrecer la gestión de historias clínicas EHR\u002FEMR, citas médicas, la adherencia del paciente al tratamiento, etc.\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">Otro despliegue de chatbots de gran repercusión llegó con el anuncio del National Health Service (NHS) del Reino Unido, que pretende probar en fase beta chatbots con \u003C\u002Fspan>\u003Cspan style=\"font-weight: 400;\">Babylon, una startup británica de telemedicina que permite videoconferencias con médicos, para «sustituir el NHS 111, una línea telefónica para casos no urgentes atendida por trabajadores de un call center que no son necesariamente profesionales sanitarios». («UK’s NHS will test Babylon’s triage chatbot to replace non-emergency hotline» 2017)\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">Entre muchas otras destaca la aplicación basada en Alexa de Amazon y creada en colaboración con el Boston Children’s Hospital, llamada KidsMD. A diferencia de las soluciones de chatbot mencionadas antes, que se basan en mensajes de texto, KidsMD es una aplicación de reconocimiento de voz que se ha probado tanto en los hogares de los pacientes como en hospitales. Los padres pueden usar la aplicación para obtener información sobre enfermedades comunes y tratamientos rápidos, todo ello supervisado por médicos y accesible en la nube. La aplicación también ayuda a los médicos a tomar notas y fotografías con metadatos cruciales mientras operan. (Bailey 2016)\u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Ca id=\"-summary\" name=\"-summary\">\u003C\u002Fa>\u003C\u002Fp>\n\u003Ch2>\u003Cspan style=\"font-weight: 400;\">Resumen\u003C\u002Fspan>\u003C\u002Fh2>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">El empeño de la salud digital por optimizar la experiencia de implicación del paciente (Patient Engagement Experience, PEx) y el proceso general de interacción con los pacientes se beneficiará sobre todo de la adopción de chatbots y aplicaciones de reconocimiento de voz en los casos en que la evaluación preliminar sea lo bastante rutinaria como para automatizarse. Las posibilidades de agilizar la comunicación entre paciente y médico existen, como hemos podido ver en ejemplos como KidsMD o la prueba de Babylon con el NHS en el Reino Unido. Cuanto más se empleen los chatbots en todo el universo digital y móvil, más familiares resultarán a los pacientes dispuestos a probarlos como alternativa. Sin embargo, el vínculo directo con los médicos no desaparecerá, y actualmente se está implantando en varios proyectos. Si la colaboración entre Babylon y el NHS tiene éxito, podrían seguirle otras grandes transformaciones de la sanidad del sector público, que estimularían a toda la industria de la salud digital. \u003C\u002Fspan>\u003C\u002Fp>\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">Aparte de los aspectos técnicos, los chatbots son un servicio y, como tal, están directamente entrelazados con los seres humanos, sus habilidades, sus miedos, sus prejuicios y su capacidad cognitiva, lo que significa que los chatbots, como cualquier servicio, deben diseñarse pensando en el ser humano. Se ha dicho que, desde la perspectiva del diseño de interfaces de usuario, el desarrollo de chatbots se ocupa menos de lo visual y se centra más en las dimensiones narrativas y conversacionales. Los chatbots plantean nuevos retos a toda la comunidad de HCI\u002FUX, que debe reorientarse hacia la nueva plataforma.
