Pokud psychiatr útočí na vlastní obor, musí mít k tomu dobrý důvod, a především podložené argumenty. Ty pak o rok později po publikaci své eseje rozvedl ve stejnojmenné knize. O co se tedy autor snaží a jaké předkládá důvody k tomu zavrhnout tehdejší koncepci duševní poruchy? Nachází se v eseji něco relevantního i pro současnou filosofii a psychiatrii?\u003C\u002Fp>\n\u003Ch2>Ontologie duševní poruchy\u003C\u002Fh2>\n\u003Cp>Duševní porucha není fyzická věc, na kterou bychom si mohli šáhnout. Nikde v těle člověka duševní poruchu nenajdeme. Pojem „duševní porucha“ je teoretický koncept, ať už zastáváme jakoukoli pozici a filozofii. A jako všechny teoretické koncepty je abstraktní a existující pouze v naší mysli, v našem jazyce. Abstrakce je jedna z funkcí přirozeného jazyka, která nám pomáhá mluvit o složitých věcech. Vědecké disciplíny jsou plné teoretických konceptů, které nemají fyzickou existenci. Fyzikální pojmy éter, černá díra, kvarky nebo Higgsův boson byly donedávna pouze teoretické konstrukty. Existovaly pouze jako předpovědi fyzické teorie. Jejich existence byla empirickou cestou ověřena až mnohem později. V případě éteru se ale ukázalo, že to, co bylo považováno za reálnou fyzickou entitu, neexistuje. Dnes pojmy jako temná energie nebo temná hmota patří stále mezi teoretické koncepty, které nám pomáhají lépe pochopit složitost a fungování vesmíru, mají \u003Cem>explanační\u003C\u002Fem> sílu, ale nemůžeme si být jisti, zda skutečně existují jako fyzické entity světa, ba zdali budeme schopni je někdy, aspoň teoreticky, v budoucnosti pozorovat.\u003C\u002Fp>\n\u003Cp>Teoretické koncepty nejsou samy o sobě nic špatného, věda se bez nich neobejde. Problém začíná tehdy, pokud něco teoretického začneme chápat tak, že ono samo má vliv a dopad na fyzický svět. Pak zaměňujeme vymyšlenou jazykovou nálepku (label), která je užitečnou zkratkou v naší mluvě, za něco s kauzální silou ovlivňovat třeba zdraví a chování člověka. Tomuto procesu, kdy se abstraktní a teoretické věci přidělí fyzická, materiální existence, se říká \u003Cem>reifikace\u003C\u002Fem> (res = věc, facere = dělat, doslova = věc-dělání, více česky = zvěcnění)\u003C\u002Fp>\n\u003Cp>I když sám to takto přímo nenazval, Szasz si myslí, že „duševní porucha“ patří do kategorie právě takovýchto reifikovaných věcí, kdy abstraktním myšlenkám začneme přisuzovat objektivní pravdu, existenci a kauzální vliv událostí. Szasz zpochybňuje fyzickou, materiální existenci toho, čemu se říká „duševní porucha“. Tvrdí, že svou užitečnost tento pojem již vyčerpal a není ničím jiným než zažitým mýtem podobně jako – argumentuje autor coby ateistický materialista – se v medicíně a ve společnosti přisuzoval kauzální vliv božstvu nebo čarodějnicím.\u003C\u002Fp>\n\u003Cp>Thomas Szasz ve svém článku zpochybňuje vše, co si tehdy, ale i v dnešní době myslíme o duševních poruchách. Autor si je vědom, že jde proti proudu tehdejšího myšlení, a proto začíná svou kritiku analýzou toho, co se v psychiatrické literatuře vůbec myslí tím, když se hovoří o duševní poruše. Jako první začíná tím, že duševní poruchu někteří další autoři redukují na projevy fyzické nemoci mozku.\u003C\u002Fp>\n\u003Ch2>Duševní porucha jako nemoc mozku\u003C\u002Fh2>\n\u003Cp>Autor nezpochybňuje, že existují různé poruchy s vlivem na myšlení a chování. Například syfilis nebo intoxikace ovlivňují náš mozek a takové poškození má dopady na duševní stav jedince. Szasz se odkazuje na dobové odborníky, kteří považovali všechny duševní poruchy za poruchy tohoto typu. Podle těchto odborníků se ale nejedná striktně řečeno o duševní poruchy, nýbrž o poruchy mozku. A všechny tyto poruchy lze, nebo bude brzy možné redukovat na fyzické a chemické procesy v těle. Pokud souhlasíme s takovým pohledem, zdá se, že nepotřebujeme samostatný pojem „duševní poruchu“. Cokoli duševní porucha je, její příčinu najdeme ve fyziochemických procesech mozku, a proto dává smysl přestat o takovém problému hovořit jako o duševním, neboť to pouze zakrývá reálnou podstatu problému a způsobuje konceptuální chaos. Místo toho bychom vždycky měli hovořit o problému mozku.\u003C\u002Fp>\n\u003Cp>Szasz s takovou interpretací ale \u003Cem>nesouhlasí\u003C\u002Fem>. Podle něj existuje speciální kategorie duševních poruch vycházející z tzv. „problému žití“ (problems in living) – osobních potřeb, motivací, názorů, aspirací, hodnot atd. –, které nelze redukovat na chemické procesy, ani lokalizovat v nějaké části mozku. Pokud lidé věří v nesmysly, neexistující věci, nebo mají abnormální a patologické zájmy, nenajdeme podle Szase příčiny v mozku. [Autor se zde mýlí, ale o tom později].\u003C\u002Fp>\n\u003Cp>Zatímco někteří autoři by redukovali duševní poruchy na fyziochemické procesy, Szasz podle mého zapadá na opačnou stranu pomyslného spektra a redukuje všechny duševní poruchy na problémy, které lze řešit psychoterapeutickou intervencí. Nejdříve se ale podívejme na to, co Szasz vůbec těmi problémy žití myslí.\u003C\u002Fp>\n\u003Ch2>Duševní porucha jako nálepka pro problémy žití\u003C\u002Fh2>\n\u003Cp>Za jednu z hlavních chyb současného společenského myšlení řadí Szasz fakt, že si myslíme, že život je v základu harmonický, jednoduchý a neproblematický. Když narazíme na nějaké těžkosti a složitosti, se kterými se nedokážeme vyrovnat, je to něco abnormálního a potřebujeme zdravotní psychiatrickou intervenci. Mohlo by se zdát, že Szasz se vydá existenciálním směrem a bude hovořit o tom, že život je existenciální problém a těžkosti tvoří nedílnou součást naší lidské existence. Ano i ne. Szasz popisuje nepřímo dva typy přístupu k duševním poruchám, které nazývám \u003Cem>etickým\u003C\u002Fem> a \u003Cem>psychoterapeutickým\u003C\u002Fem>.\u003C\u002Fp>\n\u003Ch3>Etický přístup\u003C\u002Fh3>\n\u003Cp>Etický přístup se blíží sociologické kritice psychiatrie alá Michel Foucault (viz např. můj článek \u003Ca href=\"https:\u002F\u002Fjakubferenc.cz\u002Farticle\u002Fmichel-foucault-a-otazka-konceptu-nebezpecneho-jedince-v-psychiatrii-19-stoleti\u002F\">Michel Foucault a otázka konceptu „nebezpečného jedince“ v psychiatrii 19. století\u003C\u002Fa>), v níž figurují témata jako moc, relativizace hodnot, subjektivita hodnocení toho, kdo a co je nemocný, a role psychiatra jako toho, kdo dohlíží jménem státní moci na abnormální jedince a koná jakousi hygienickou očistu společnosti.\u003C\u002Fp>\n\u003Cp>Szaszův přístup, který se mi tedy jeví jako \u003Cem>problematizující etickou rovinu psychiatrie\u003C\u002Fem>, kritizuje základní předpoklad nastavení společnosti. A to ten, že život za normálních okolností je harmonický a jakkoukoli odchylku chápe společnost jako deviaci od nějaké normy. A tuto deviaci je potřeba vyřešit, odstranit a ideálně jí do budoucna preventivně předcházet, aby neohrožovala „normální“ většinu populace. Kdo ale definuje, co je normální a jaké normy by se měly následovat? Ve většině případů to je právě osoba psychiatra, který prostřednictvím své socializace v dané společnosti musí interpretovat to, co se jeví v mluveném projevu daného jedince abnormálně.\u003C\u002Fp>\n\u003Cp>Psychiatr je podle názorů Szasze společností stanovený expert, který vyhodnocuje abnormalitu chování, názorů, myšlenek daného pacienta. Psychiatr se stává kritikem a interpretem životního příběhu a slovního projevu potenciálního pacienta. Psychiatr toto nečiní na zelené louce. Ale nevyhnutelně \u003Cem>interpretuje\u003C\u002Fem> chování pacienta na základě své životní zkušenosti ve velmi specifickém sociálním a kulturním kontextu a na základě svého světonázoru toho, co považuje za realitu, normu, standard.\u003C\u002Fp>\n\u003Cp>Ve článku nacházíme zajímavý termín pozorovatel-účastník (ang. participant observer), kterým popisuje, že psychiatr není pouze analytický a objektivní pozorovatel, nýbrž sám vstupuje do procesu definice toho, kdo a jak je nemocný. Podobně si můžeme představit literárního kritika, který svou povahou, znalostmi, ale i vkusem si vybírá, na co se v dané knize, básni nebo románu zaměří. Analogie to není přesná, protože psychiatrie má statisticky a vědecky podložené tabulky a metody, jak vyhodnotit stav pacienta, ale není možné od psychiatra očekávat, že jeho osobní a subjektivní hodnocení nevstupuje do zdánlivě objektivního procesu vyhodnocení duševní poruchy. Ostatně několik psychiatrů se často neshodne na jedné diagnóze, to by nám samo o sobě mělo naznačit, že psychiatrie rozhodně není exaktní věda; ale tou není ani medicína.\u003C\u002Fp>\n\u003Cp>Právě tento neoprávněný punc exaktnosti Szasz na psychiatrii kritizuje. Jak autor píše: „žádná lidská aktivita není oprostěná od lidských hodnot“\u003Csup>\u003Csup>\u003Ca id=\"post-934-footnote-ref-1\" href=\"#post-934-footnote-1\">[1]\u003C\u002Fa>\u003C\u002Fsup>\u003C\u002Fsup>. To by však znamenalo, že i medicína samotná je prosycena lidskými hodnotami, které do medicíny vnáší prvek osobního vkusu, úsudku, tedy subjektivity. Toho si je vědom i autor, proto musí argumentovat, že sice i medicína je svázána s etickými a subjektivními hodnoceními, ale psychiatrie mnohem více\u003Csup>\u003Csup>\u003Ca id=\"post-934-footnote-ref-2\" href=\"#post-934-footnote-2\">[2]\u003C\u002Fa>\u003C\u002Fsup>\u003C\u002Fsup>.\u003C\u002Fp>\n\u003Ch3>Etický přístup a problém komunikace\u003C\u002Fh3>\n\u003Cp>Asi nejvíce kontroverzní moment v eseji nacházíme v pasážích, kde se Szasz snaží redukovat všechny duševní problémy, které není možné identifikovat jako fyziochemické (tedy problémy mozku), na problémy v komunikaci – v tom, jak člověk mluví, co vyjadřuje, jak reaguje. Když psychiatr nerozumí něčí řeči nebo ji považuje za „nevhodnou“, označí to za „abnormální“, a psychiatři to lékařsky kodifikují jako „symptom“ nějaké nemoci. Szasz píše:\u003C\u002Fp>\n\u003Cblockquote>\u003Cp>„mental illnesses are in fact communications expressing unacceptable ideas, often framed, moreover, in an unusual idiom.“\u003C\u002Fp>\u003C\u002Fblockquote>\n\u003Cp>Jistě, v historii nacházíme mnoho příkladů, kdy vyjádření podivných názorů bylo považováno za projev duševní poruchy. Například v Sovětském svazu se mnoho lidí ocitlo nejen ve vězení, ale také na psychiatrických klinikách jen proto, že měli odvahu vyjádřit politicky nekorektní názory. Vzpomeňme si na 1984 od George Orwella, v němž hlavní postava Winston Smith je poslán do Místnosti 101 na mučení a převýchovu kvůli svým názorům. Psychiatrie z tohoto cynického pohledu může být chápána jako nástroj společenské kontroly – podobně jako thoughtcrime v Orwellově románu.\u003C\u002Fp>\n\u003Cp>Ale je možné všechny podivné věci, které lidé vysloví, redukovat na to, že psychiatr vyhodnotí něco, co říkáme, za divné a nepřípustné? Vždyť abnormální jazykové projevy jsou legitimním symptomem závažného zdravotního problému. Různé léze na temporálním nebo frontálním laloku mohou způsobit bludy a halucinace. Szasz se tedy mýlí, když tvrdí, že tyto podivné názory a přesvědčení, že někdo je Napoleon nebo je perzekuován CIA, nemohou být způsobené a vysvětlené defektem nebo nemocí nervového systému.\u003Csup>\u003Csup>\u003Ca id=\"post-934-footnote-ref-3\" href=\"#post-934-footnote-3\">[3]\u003C\u002Fa>\u003C\u002Fsup>\u003C\u002Fsup>\u003C\u002Fp>\n\u003Cp>Najít příčinu nějakého bludu je komplexní úkol. Je o to složitější, pokud chceme být přesní.\u003C\u002Fp>\n\u003Cp>Na jednu stranu to, že psychiatr musí na základě své vlastní interpretace posoudit, že se s pacientem něco děje, protože pacient tvrdí nesmysly, je pravdivé tvrzení. Psychiatr by pak s pomocí snímků z magnetické rezonance a s vyjádřením neurologa prohlásil diagnózu, že příčinou bludu je léze na mozku. Na druhou stranu nám už psychiatrie nedokáže vysvětlit, proč se ta \u003Cem>konkrétní\u003C\u002Fem> léze na \u003Cem>konkrétním\u003C\u002Fem> místě projevuje tímto \u003Cem>konkrétním\u003C\u002Fem> způsobem. Jinak řečeno, proč se fyzická porucha mozku projevuje jako blud o perzekuci CIA a ne třeba perzekuce čínskou komunistickou stranou?\u003C\u002Fp>\n\u003Cp>To je otázka, na kterou nemám odpověď. A pochybuji, že má na ní odpověď Szasz nebo psychiatrie samotná. Zdálo by se mi rozumné analyzovat anamnézu takového pacienta a zjistit, zda a jak se zajímal o CIA nebo americkou politiku a historii. Dokážu si představit, že ale existují případy, kdy například radikální ateista najednou spatřil panenku Marii nebo Ježíše, přestože intelektem a názory byl přesvědčen o pravém opaku. Co pak s takovým člověkem? Můžeme jeho bludy (předpokládejme, že se jedná o bludy a ne pravé zjevení) chápat tak, že jsou založené na jeho znalostech a zájmech? A i kdyby tomu tak bylo, \u003Cem>zdá se mi, že v následujících 20–30 letech\u003C\u002Fem> budeme někdy schopni vysvětlit na fyziochemické úrovni kauzální vztah léze a vizuálně-auditorního obsahu bludu. Možná budeme schopni najít více asociací, ale najít asociaci neboli neurální korelát obsahu bludu neznamená vysvětlit příčinu vědecky – ve formě mechanistického modelu příčin, kde krok za krokem dokážeme vysvětlit příčinu a predikovat to, co a kde způsobí.\u003C\u002Fp>\n\u003Cp>Szasz zároveň ignoruje problémy, které dnes řadíme do duševních problémů a jsou vyvolány pouze v konkrétních případech u konkrétních lidí. Většinou v nějakém extrémně vypjatém momentu života. Například různé úzkostlivé poruchy nebo mánie (trichotillománie, kleptománie apod.) jsou vyvolány ve stresové situaci. Stres není jejich skutečnou příčinou (a na základě předchozího odstavce to může být velmi filosofická otázka), hovoříme o existujících predispozicích. Avšak bez konkrétního spouštěče by se predispozice nikdy nemusela projevit. Jaký je toto problém? Čistě mozku? Čistě společenský? Čistě osobnostní? Zdá se, že někde mezi, a proto zase jít druhou stranou, a redukovat všechny duševní poruchy na problémy mozku se mi jeví jako chyba. Úzkost vyvolanou vnějším spouštěčem nikdy nenajdeme v samotném mozku. Je to duševní problém, který je emergujícím projevem v chování na základě interakce genů, osobnostního nastavení daného člověka (úzkostlivá povaha) a společenských vlivů (např. příprava na zkoušku). V tomto případě se Szasz nemůže opírat o redukci „směrem dolů“ k mozku, ani o redukci „směrem nahoru“, tedy k čistě osobnostnímu nastavení člověka a vlivům společnosti.\u003C\u002Fp>\n\u003Cp>Takže ano, Szasz má pravdu v tom, že psychiatrie užívá do jisté, a pro Szasze příliš velké míry subjektivní metody pro vyhodnocení toho, zda je pacient duševně nemocný. Tato hodnocení jsou nutně podmíněna sociálně, kulturně, právně, jinými slovy do zdánlivě exaktního hodnocení odborníka s titulem z medicíny se vplíží hodnotové soudy, a tudíž etické soudy. A ano, dopátrat se konkrétní příčiny bludných přesvědčení může být vědecky i filosoficky nemožný úkol. Přesto autorův názor, že bludy nemohou být způsobeny problémy na mozku, je z největší pravděpodobnosti nesprávný.\u003C\u002Fp>\n\u003Ch3>Problémy žití a psychoterapeutický přístup\u003C\u002Fh3>\n\u003Cp>Kde s autorem souhlasím je názor, že určitá podmnožina problémů, které jsou dnes označovány za duševní poruchy, lze zařadit do existenciální kategorie, jak se vyrovnat se základní otázkou života: jak žít. Tento typ problémů nazývám psychoterapeutickým přístupem k problémům žití proto, že chápu Szaszův argument tak, že určitá forma psychoterapie nebo „koučingu“ jsou správné metody pro léčení těchto existenciálních problémů, které medicína nesprávně, podle Szasze, si dovoluje nazývat duševními poruchami.\u003C\u002Fp>\n\u003Cp>Psychoterapeutický přístup se podle mě orientuje na podobná témata, která nacházíme v existenciálně laděné psychoterapii, jejíž kořeny sahají k fenomenologii vycházející z díla německého filosofa Martina Heideggera. Heidegger a další se zabývali myšlenkou, že základem naší lidské existence je fakt, že jsme do světa \u003Cem>vrženi:\u003C\u002Fem>\u003C\u002Fp>\n\u003Cblockquote>\u003Cp>„Κ &#8218;bytí ve světě* však patří, že &#8211; sobě samé- mu vydáno -je vždy již vrženo \u003Cem>do určitého světa\u003C\u002Fem>”\u003C\u002Fp>\n\u003Cp>(Bytí a čas, §41. Bytí pobytu jako starost, s. 221)\u003C\u002Fp>\u003C\u002Fblockquote>\n\u003Cp>Tato Vrženost (ang. Throwness, něm. Geworfenheit) označuje danost naší existence. Ocitli jsme se ve světě, bez našeho přičinění, a patříme do určité kultury, vztahů a dějinných okamžiků. Francouz Merleau-Ponty by dodal, že máme i určité tělesné dispozice, které formují náš vztah k druhým lidem a ke světu obecně. Jinak řečeno, rodíme se do života, který jsme si nezvolili, a musíme v něm nějak existovat. Čím složitější společnost, ve které žijeme, tím více různorodých problémů musíme řešit. Někteří se vypořádávají se životem lépe, někteří hůře. Někteří začínají od sebe, druzí vidí primární problémy ve společenských strukturách a politických proměnných, které jsme taktéž zdědili.\u003C\u002Fp>\n\u003Cp>Szasz se považuje za radikálního individualistu. Anebo aspoň tak se mi jeví. Protože jeho psychoterapeutickým přístupem k řešení existenciálních problémů je radikální zodpovědnost za sebe sama založená, slovy autora, na racionálním porozumění vlastní situace a následování vhodných kroků založených na tomto porozumění. Szasz věří, že veškeré řešení našich problémů máme na dosah ruky a pouze naše individuální zodpovědnost za naše činy povede k řešení našich existenciálních problémů a našich pseudo duševních poruch.\u003C\u002Fp>\n\u003Cp>Szasz zcela odmítá vidět původ komplexity a obtížnosti našich životů na vyšších strukturách, například právě politické a společenské struktury. Ve svém článku ale nevysvětluje, proč tyto vyšší struktury ignoruje.\u003C\u002Fp>\n\u003Cp>Autor sice explicitně odmítá, že by zakládal nějakou psychoterapeutickou školu. Přesto nabízí konkrétní recept pro lepší duševní zdraví. \u003Cem>Problémem je podle něj volba\u003C\u002Fem>. A jaké volby podnikáme během našeho života ovlivňují naše duševní zdraví. Pokud volíme \u003Cem>dobře\u003C\u002Fem>, jsme lépe připraveni se vypořádat se složitostí našich životů. Co znamená dobře? Jak už bylo řečeno, naše volby musí být založené na racionálním přístupu a musíme mít co nejlepší porozumění naší životní situaci, protože právě tehdy budeme s to činit ne náhodné, ale racionální volby, které by snad měly vést k lepšímu životu a duševní pohodě. Naše duševní zdraví je přímo úměrné kvalitě našich životních voleb, a jak racionální tyto volby děláme.\u003C\u002Fp>\n\u003Ch2>Shrnutí\u003C\u002Fh2>\n\u003Cp>Thomas Szasz ve svém vlivném článku The Myth of Mental illness z roku 1960 tvrdí, že duševní poruchy jsou mýtem a jako takové neexistují. Duševní poruchy totiž můžeme buďto redukovat na fyziochemické problémy v mozku (a tím jsou tedy problémy mozku), anebo se jedná o pseudo duševní poruchy, které jsou spíše problémy žití, neboli vyrovnávání se s komplexitou života.\u003C\u002Fp>\n\u003Cp>Tyto pseudo duševní poruchy dál autor analyzuje dvěma přístupy, které nazývám \u003Cem>etický\u003C\u002Fem> a \u003Cem>psychoterapeutický\u003C\u002Fem>. První z nich poukazuje na etický a hodnotově relativizující rozměr práce psychiatrů, kde psychiatrův světonázor, kulturní pozadí a subjektivní dojmy často rozhodují o tom, zda daná slovní promluva bude vyhodnocena jako duševní porucha, anebo nikoli. Autor tímto poukazuje na to, že medicína obecně a psychiatrie konkrétně nejsou hodnotově neutrální exaktní vědy, nýbrž jsou nevyhnutelně odkázané na subjektivní soudy psychiatrů.\u003C\u002Fp>\n\u003Cp>Psychoterapeutickým přístupem myslím typ kritiky oboru psychiatrie, kde autor duševní poruchy vidí jako existenciální problémy žití ve společnosti. Autor sice explicitně odmítá, že by chtěl vytvořit novou školu psychoterapie, přesto nabízí analýzu existenciálních problémů a možnou nápravu, která dle mého názoru představuje formu psychoterapeutické analýzy a intervence. Konkrétně si autor myslí, že lidé mohou omezit duševní problémy způsobené komplexitou života, pokud budou činit racionální životní volby založené na porozumění své životní situace. Autor klade vinu duševních problémů na špatných individuálních rozhodnutích jedince. Tento radikální individualismus ignoruje případné vlivy vyšších struktur – společnost, politické zřízení, historické souvislosti apod. Podle autora je naše duševní zdraví přímo úměrné kvalitě našich životních voleb, a jak racionální tyto volby děláme. Szaszova kritika tak zůstává výzvou k reflexi samotných základů psychiatrie – tedy k otázce, zda „duševní nemoc“ označuje skutečnou entitu, nebo spíše soubor našich kulturních soudů o tom, jak by měl člověk žít. If a psychiatrist attacks his own field, he must have a good reason for it and, above all, well-founded arguments. He developed them a year after publishing the essay, in a book of the same name. So what is the author trying to do, and what reasons does he put forward for rejecting the conception of mental disorder of his time? Is there anything in the essay that is relevant to today’s philosophy and psychiatry too?\u003C\u002Fp>\n\u003Ch2>The ontology of mental disorder\u003C\u002Fh2>\n\u003Cp>A mental disorder is not a physical thing that we could touch. We will not find a mental disorder anywhere in the human body. The term “mental disorder” is a theoretical concept, whatever position and philosophy we hold. And like all theoretical concepts, it is abstract and exists only in our mind, in our language. Abstraction is one of the functions of natural language that helps us talk about complex things. Scientific disciplines are full of theoretical concepts that have no physical existence. The physical notions of the ether, the black hole, quarks or the Higgs boson were until recently merely theoretical constructs. They existed only as predictions of a physical theory. Their existence was verified empirically only much later. In the case of the ether, however, it turned out that what had been considered a real physical entity does not exist. Today, notions such as dark energy or dark matter still belong among the theoretical concepts that help us better understand the complexity and workings of the universe; they have \u003Cem>explanatory\u003C\u002Fem> power, but we cannot be sure whether they really exist as physical entities of the world, or indeed whether we will ever be able, at least in theory, to observe them in the future.\u003C\u002Fp>\n\u003Cp>Theoretical concepts are nothing bad in themselves; science cannot do without them. The problem begins when we start to understand something theoretical as having, by itself, an influence on and an impact on the physical world. We then mistake an invented linguistic label, which is a useful shorthand in our speech, for something with the causal power to affect, say, a person’s health and behaviour. This process, in which abstract and theoretical things are assigned a physical, material existence, is called \u003Cem>reification\u003C\u002Fem> (res = thing, facere = to make, literally = thing-making, in plainer English = turning into a thing)\u003C\u002Fp>\n\u003Cp>Although he did not call it that himself, Szasz thinks that “mental disorder” belongs precisely to this category of reified things, in which we begin to attribute objective truth, existence and causal influence over events to abstract ideas. Szasz questions the physical, material existence of what is called “mental disorder”. He claims that the concept has outlived its usefulness and is nothing but an entrenched myth, just as – argues the author as an atheist materialist – medicine and society once attributed causal influence to deities or witches.\u003C\u002Fp>\n\u003Cp>In his article Thomas Szasz questions everything we thought about mental disorders then, and still think today. The author is aware that he is going against the current of the thinking of his time, and so he begins his critique by analysing what the psychiatric literature actually means when it speaks of mental disorder. He starts with the fact that some other authors reduce mental disorder to the manifestations of a physical disease of the brain.\u003C\u002Fp>\n\u003Ch2>Mental disorder as a disease of the brain\u003C\u002Fh2>\n\u003Cp>The author does not dispute that there are various disorders that affect thinking and behaviour. Syphilis or intoxication, for example, affect our brain, and such damage has an impact on the individual’s mental state. Szasz refers to the experts of his day who considered all mental disorders to be disorders of this type. According to these experts, however, these are, strictly speaking, not mental disorders but disorders of the brain. And all these disorders can be, or soon will be, reduced to physical and chemical processes in the body. If we agree with such a view, it seems that we do not need a separate concept of “mental disorder”. Whatever a mental disorder is, we will find its cause in the physicochemical processes of the brain, and so it makes sense to stop speaking of such a problem as mental, since that only obscures the real nature of the problem and causes conceptual chaos. Instead, we should always speak of a problem of the brain.\u003C\u002Fp>\n\u003Cp>Szasz, however, does \u003Cem>not agree\u003C\u002Fem> with such an interpretation. According to him, there is a special category of mental disorders arising from so-called “problems in living” – personal needs, motivations, opinions, aspirations, values, etc. – which cannot be reduced to chemical processes, nor located in some part of the brain. If people believe in nonsense, in things that do not exist, or have abnormal and pathological interests, then according to Szasz we will not find the causes in the brain. [The author is wrong here, but more on that later].\u003C\u002Fp>\n\u003Cp>While some authors would reduce mental disorders to physicochemical processes, Szasz, in my view, falls on the opposite side of the imaginary spectrum and reduces all mental disorders to problems that can be solved by psychotherapeutic intervention. But first let us look at what Szasz actually means by these problems in living.\u003C\u002Fp>\n\u003Ch2>Mental disorder as a label for problems in living\u003C\u002Fh2>\n\u003Cp>Szasz counts among the main errors of contemporary social thought the fact that we think life is fundamentally harmonious, simple and unproblematic. When we come across some difficulties and complexities that we cannot cope with, it is something abnormal and we need medical, psychiatric intervention. It might seem that Szasz will set off in an existential direction and talk about life being an existential problem and difficulties forming an integral part of our human existence. Yes and no. Szasz indirectly describes two types of approach to mental disorders, which I call \u003Cem>ethical\u003C\u002Fem> and \u003Cem>psychotherapeutic\u003C\u002Fem>.\u003C\u002Fp>\n\u003Ch3>The ethical approach\u003C\u002Fh3>\n\u003Cp>The ethical approach comes close to a sociological critique of psychiatry à la Michel Foucault (see e.g. my article \u003Ca href=\"https:\u002F\u002Fjakubferenc.cz\u002Farticle\u002Fmichel-foucault-a-otazka-konceptu-nebezpecneho-jedince-v-psychiatrii-19-stoleti\u002F\">Michel Foucault and the Concept of the “Dangerous Individual” in 19th-Century Psychiatry\u003C\u002Fa>), in which themes such as power, the relativisation of values, the subjectivity of judging who and what is ill, and the role of the psychiatrist as the one who, in the name of state power, watches over abnormal individuals and carries out a kind of hygienic cleansing of society all figure.\u003C\u002Fp>\n\u003Cp>Szasz’s approach, which thus appears to me as \u003Cem>problematising the ethical dimension of psychiatry\u003C\u002Fem>, criticises a basic assumption on which society is set up. Namely, that life under normal circumstances is harmonious, and that society understands any departure from it as a deviation from some norm. And this deviation needs to be dealt with, removed and ideally prevented in the future, so that it does not threaten the “normal” majority of the population. But who defines what is normal and which norms should be followed? In most cases it is precisely the psychiatrist, who, through his socialisation in the given society, has to interpret what appears abnormal in the speech of the individual in question.\u003C\u002Fp>\n\u003Cp>The psychiatrist, in Szasz’s view, is an expert appointed by society who assesses the abnormality of the given patient’s behaviour, opinions and thoughts. The psychiatrist becomes a critic and interpreter of the life story and verbal expression of the potential patient. The psychiatrist does not do this from scratch. Rather, he inevitably \u003Cem>interprets\u003C\u002Fem> the patient’s behaviour on the basis of his own life experience in a very specific social and cultural context, and on the basis of his worldview of what he considers reality, the norm, the standard.\u003C\u002Fp>\n\u003Cp>In the article we find the interesting term participant observer, with which he describes how the psychiatrist is not merely an analytical and objective observer, but himself enters into the process of defining who is ill and how. Similarly, we can imagine a literary critic who, through his character, his knowledge, but also his taste, chooses what to focus on in a given book, poem or novel. The analogy is not exact, because psychiatry has statistically and scientifically grounded tables and methods for assessing a patient’s condition, but we cannot expect a psychiatrist’s personal and subjective judgement not to enter into the seemingly objective process of assessing a mental disorder. After all, several psychiatrists often do not agree on a single diagnosis; that in itself should suggest to us that psychiatry is certainly not an exact science; but neither is medicine.\u003C\u002Fp>\n\u003Cp>It is precisely this unwarranted stamp of exactness that Szasz criticises in psychiatry. As the author writes: “no human activity is devoid of human value”\u003Csup>\u003Csup>\u003Ca id=\"post-934-footnote-ref-1\" href=\"#post-934-footnote-1\">[1]\u003C\u002Fa>\u003C\u002Fsup>\u003C\u002Fsup>. That, however, would mean that medicine itself is also permeated by human values, which bring into medicine an element of personal taste and judgement, that is, of subjectivity. The author is aware of this too, which is why he has to argue that although medicine is also bound up with ethical and subjective judgements, psychiatry is much more so\u003Csup>\u003Csup>\u003Ca id=\"post-934-footnote-ref-2\" href=\"#post-934-footnote-2\">[2]\u003C\u002Fa>\u003C\u002Fsup>\u003C\u002Fsup>.\u003C\u002Fp>\n\u003Ch3>The ethical approach and the problem of communication\u003C\u002Fh3>\n\u003Cp>Perhaps the most controversial moment in the essay is found in the passages where Szasz tries to reduce all mental problems that cannot be identified as physicochemical (that is, as problems of the brain) to problems of communication – of how a person speaks, what they express, how they react. When a psychiatrist does not understand someone’s speech or considers it “inappropriate”, he labels it “abnormal”, and psychiatrists codify it medically as a “symptom” of some disease. Szasz writes:\u003C\u002Fp>\n\u003Cblockquote>\u003Cp>“mental illnesses are in fact communications expressing unacceptable ideas, often framed, moreover, in an unusual idiom.”\u003C\u002Fp>\u003C\u002Fblockquote>\n\u003Cp>Certainly, we find many examples in history where the expression of strange opinions was considered a manifestation of mental disorder. In the Soviet Union, for example, many people ended up not only in prison but also in psychiatric clinics simply because they had the courage to express politically incorrect opinions. Let us recall George Orwell’s 1984, in which the main character, Winston Smith, is sent to Room 101 for torture and re-education because of his opinions. From this cynical point of view, psychiatry can be understood as an instrument of social control – much like thoughtcrime in Orwell’s novel.\u003C\u002Fp>\n\u003Cp>But is it possible to reduce all the strange things people say to a psychiatrist judging something we say to be strange and unacceptable? After all, abnormal linguistic expressions are a legitimate symptom of a serious health problem. Various lesions of the temporal or frontal lobe can cause delusions and hallucinations. Szasz is therefore wrong when he claims that these strange opinions and beliefs, that someone is Napoleon or is being persecuted by the CIA, cannot be caused and explained by a defect or disease of the nervous system.\u003Csup>\u003Csup>\u003Ca id=\"post-934-footnote-ref-3\" href=\"#post-934-footnote-3\">[3]\u003C\u002Fa>\u003C\u002Fsup>\u003C\u002Fsup>\u003C\u002Fp>\n\u003Cp>Finding the cause of a delusion is a complex task. All the more so if we want to be precise.\u003C\u002Fp>\n\u003Cp>On the one hand, it is a true claim that the psychiatrist has to judge, on the basis of his own interpretation, that something is happening to the patient because the patient is saying nonsensical things. The psychiatrist would then, with the help of magnetic resonance images and a neurologist’s opinion, pronounce the diagnosis that the cause of the delusion is a lesion in the brain. On the other hand, psychiatry can no longer explain to us why that \u003Cem>particular\u003C\u002Fem> lesion in that \u003Cem>particular\u003C\u002Fem> place manifests itself in this \u003Cem>particular\u003C\u002Fem> way. In other words, why does a physical disorder of the brain manifest itself as a delusion of persecution by the CIA and not, say, of persecution by the Chinese Communist Party?\u003C\u002Fp>\n\u003Cp>That is a question I have no answer to. And I doubt that Szasz or psychiatry itself has an answer to it. It would seem reasonable to me to analyse the history of such a patient and find out whether and how he was interested in the CIA or in American politics and history. But I can imagine that there are cases where, for example, a radical atheist suddenly saw the Virgin Mary or Jesus, even though in his intellect and opinions he was convinced of the very opposite. What then with such a person? Can we understand his delusions (let us assume that they are delusions and not a true revelation) as being based on his knowledge and interests? And even if that were so, \u003Cem>it seems to me that in the next 20–30 years\u003C\u002Fem> we will at some point be able to explain, at the physicochemical level, the causal relationship between the lesion and the visual and auditory content of the delusion. Perhaps we will be able to find more associations, but finding an association, or a neural correlate, of the content of a delusion does not mean explaining its cause scientifically – in the form of a mechanistic model of causes, in which we can explain the cause step by step and predict what it will bring about and where.\u003C\u002Fp>\n\u003Cp>At the same time, Szasz ignores problems that we today classify as mental problems and that are triggered only in specific cases in specific people. Mostly at some extremely tense moment in life. For example, various anxiety disorders or manias (trichotillomania, kleptomania, etc.) are triggered in a stressful situation. Stress is not their real cause (and in the light of the previous paragraph this may be a very philosophical question); we speak of existing predispositions. Yet without a specific trigger, the predisposition might never manifest itself. What kind of problem is this? Purely of the brain? Purely social? Purely of personality? It seems to be somewhere in between, and so going the other way, too, and reducing all mental disorders to problems of the brain appears to me to be a mistake. We will never find anxiety triggered by an external trigger in the brain itself. It is a mental problem that is an emergent manifestation in behaviour, arising from the interaction of genes, the personality make-up of the person in question (an anxious nature) and social influences (e.g. preparing for an exam). In this case Szasz can rely neither on a reduction “downwards” to the brain, nor on a reduction “upwards”, that is, to the purely personal make-up of the person and the influences of society.\u003C\u002Fp>\n\u003Cp>So yes, Szasz is right that psychiatry uses subjective methods, to a certain extent and, for Szasz, to too great an extent, to assess whether a patient is mentally ill. These assessments are necessarily conditioned socially, culturally, legally; in other words, value judgements, and therefore ethical judgements, creep into the seemingly exact assessment of an expert with a medical degree. And yes, tracking down the specific cause of delusional beliefs may be a scientifically as well as philosophically impossible task. Nevertheless, the author’s view that delusions cannot be caused by problems in the brain is in all likelihood incorrect.\u003C\u002Fp>\n\u003Ch3>Problems in living and the psychotherapeutic approach\u003C\u002Fh3>\n\u003Cp>Where I agree with the author is in the view that a certain subset of the problems that are today labelled mental disorders can be placed in an existential category: how to cope with the basic question of life, namely how to live. I call this type of problem the psychotherapeutic approach to problems in living because I understand Szasz’s argument to say that some form of psychotherapy or “coaching” is the right method for treating these existential problems, which medicine, according to Szasz, wrongly takes the liberty of calling mental disorders.\u003C\u002Fp>\n\u003Cp>The psychotherapeutic approach, in my view, is oriented towards themes similar to those we find in existentially minded psychotherapy, whose roots go back to the phenomenology that grew out of the work of the German philosopher Martin Heidegger. Heidegger and others dealt with the idea that the basis of our human existence is the fact that into the world we are \u003Cem>thrown:\u003C\u002Fem>\u003C\u002Fp>\n\u003Cblockquote>\u003Cp>“But to ‘being-in-the-world’ belongs the fact that – delivered over to itself – it has always already been thrown \u003Cem>into a certain world\u003C\u002Fem>”\u003C\u002Fp>\n\u003Cp>(Bytí a čas, §41. Bytí pobytu jako starost, p. 221)\u003C\u002Fp>\u003C\u002Fblockquote>\n\u003Cp>This thrownness (Ger. Geworfenheit) designates the givenness of our existence. We found ourselves in the world without our doing, and we belong to a particular culture, to relationships and to historical moments. The Frenchman Merleau-Ponty would add that we also have certain bodily dispositions that shape our relationship to other people and to the world in general. In other words, we are born into a life we did not choose, and we have to exist in it somehow. The more complex the society we live in, the more varied the problems we have to deal with. Some cope with life better, some worse. Some start with themselves; others see the primary problems in social structures and political variables, which we have likewise inherited.\u003C\u002Fp>\n\u003Cp>Szasz considers himself a radical individualist. Or at least that is how he appears to me. For his psychotherapeutic approach to solving existential problems is a radical responsibility for oneself, based, in the author’s words, on a rational understanding of one’s own situation and on following the appropriate steps based on that understanding. Szasz believes that the solution to all our problems is within reach, and that only our individual responsibility for our actions will lead to the solution of our existential problems and our pseudo-mental disorders.\u003C\u002Fp>\n\u003Cp>Szasz flatly refuses to see the origin of the complexity and difficulty of our lives in higher structures, for example political and social structures. In his article, however, he does not explain why he ignores these higher structures.\u003C\u002Fp>\n\u003Cp>The author does explicitly deny that he is founding any school of psychotherapy. Even so, he offers a concrete recipe for better mental health. \u003Cem>The problem, according to him, is choice\u003C\u002Fem>. And the choices we make during our lives affect our mental health. If we choose \u003Cem>well\u003C\u002Fem>, we are better prepared to cope with the complexity of our lives. What does well mean? As has already been said, our choices must be based on a rational approach, and we must have the best possible understanding of our life situation, because only then will we be able to make not random but rational choices, which should, hopefully, lead to a better life and mental well-being. Our mental health is directly proportional to the quality of our life choices and to how rationally we make them.\u003C\u002Fp>\n\u003Ch2>Summary\u003C\u002Fh2>\n\u003Cp>In his influential 1960 article The Myth of Mental Illness, Thomas Szasz claims that mental disorders are a myth and as such do not exist. For mental disorders can either be reduced to physicochemical problems in the brain (and so they are problems of the brain), or they are pseudo-mental disorders, which are rather problems in living, that is, of coping with the complexity of life.\u003C\u002Fp>\n\u003Cp>The author goes on to analyse these pseudo-mental disorders through two approaches, which I call \u003Cem>ethical\u003C\u002Fem> and \u003Cem>psychotherapeutic\u003C\u002Fem>. The first points to the ethical, value-relativising dimension of psychiatrists’ work, in which the psychiatrist’s worldview, cultural background and subjective impressions often decide whether a given utterance will be assessed as a mental disorder or not. The author thereby points out that medicine in general and psychiatry in particular are not value-neutral exact sciences, but are inevitably dependent on the subjective judgements of psychiatrists.\u003C\u002Fp>\n\u003Cp>By the psychotherapeutic approach I mean a type of critique of the field of psychiatry in which the author sees mental disorders as existential problems of living in society. Although the author explicitly denies wanting to create a new school of psychotherapy, he nevertheless offers an analysis of existential problems and a possible remedy which, in my opinion, represents a form of psychotherapeutic analysis and intervention. Specifically, the author thinks that people can reduce the mental problems caused by the complexity of life if they make rational life choices based on an understanding of their life situation. The author places the blame for mental problems on the individual’s bad individual decisions. This radical individualism ignores the possible influences of higher structures – society, the political system, historical circumstances, etc. According to the author, our mental health is directly proportional to the quality of our life choices and to how rationally we make them. Szasz’s critique thus remains a call to reflect on the very foundations of psychiatry – that is, on the question of whether “mental illness” denotes a real entity, or rather a set of our cultural judgements about how a person ought to live.
